Meet the Team
Daniela Fawson
Physician Assistant
Daniela is a Physician Assistant who was born and spent her early childhood in Chile. She has been in the US since she was 12 years old when her family moved to Kentucky and she learned to speak southern English. She loved growing up in the horse capital of the world, but has recently fallen in love with Utah.
She has always had an interest in medicine from a young age due to the influence of her mother who was a Midwife. She went to school at Eastern Kentucky University for her Bachelor’s in Medical Laboratory Science. She then went to the University of Kentucky and obtained a Master’s in Physician Assistant Studies.
Daniela speaks fluent Spanish and enjoys getting to use it with Hispanic patients. One of her top priorities is making sure patients feel listened to and receive what they need from their visit.
Outside of work she is married to her husband, David, and together they have 2 young children who keep her busy and laughing. She enjoys sewing, needlework, gardening, and anything dark chocolate.
LaVal B. Jensen
Chairman of the Board / CEO
LaVal is a results-oriented, accomplished Executive with a broad background in business, healthcare and human services and extensive operational and administrative expertise. He has significant experience in single and multi-service corporations, management companies, partnerships, joint ventures, mergers and acquisitions, with a broad knowledge of the healthcare and the managed care industry. LaVal’s vision and leadership skills, including strong corporate, organizational, administrative and management skills, are demonstrated by his successful and proven track record in business, both existing business and start-ups and propelling them to multi-million-dollar revenues.
LaVal is a “success-driven” leader with an innate ability to conceive, create, implement, and evaluate, utilizing traditional and entrepreneurial concepts to logically reach desired goals.
LaVal is married, with an amazing family including two sons and their Spouses, and 5 beautiful grandchildren. LaVal loves to spend time with his family, including physical fitness, health optimization, and boating.
Psychotherapy Team and Services
The relationship between a therapist and client has proven to be one of the most important elements to successful outcomes in therapy. For this reason, Affinity Healthcare will strive to build our practice with personable and competent therapists whom our patients can trust. Meet them below!
Parker Donner Ph.D.
Psychologist
Dr. Donner has been a licensed psychologist since 2022. He obtained his undergraduate degree in psychology with a minor in Spanish from the University of North Texas. He obtained his Masters and Ph.D. in counseling psychology from Texas Tech University. He specializes in diagnostic assessments and the implementation of evidence-based treatments for mental health disorders (e.g. depression, anxiety, OCD, PTSD).
Dr. Donner provides a friendly, caring environment and is motivated to help his clients find healing and real solutions through psychotherapy. He has extensive experience in the diagnosis of mental health disorders, working with family members of people with dementia, and behavioral planning to reduce the risk of violence.
Dr. Donner is married and has 3 children. He enjoys reading, football, and working outside. Dr. Donner is licensed in Texas and Kansas and has interjurisdictional license to provide therapy services in PSYPACT participating states (https://psypact.org/mpage/psypactmap) including Utah, Texas, Idaho, and Wyoming. He is currently accepting new clients via teletherapy, providing a safe and effective space to meet.
Corey Jensen CSW/LMSW
Social Worker
Corey has been practicing as a licensed social worker since 2022. He received his undergraduate degree in Psychology from Southern Virginia University and his master’s in social work from Baylor University. He specializes in clinical social work with certifications in trauma response and integrated behavioral health (i.e., behavior change congruent with primary healthcare goals).
Corey is able to quickly connect with clients and establish strong rapport that provides the foundation for successful psychotherapy. What he finds most rewarding in his work is engaging with his clients and seeing the improvements that they can make when they participate in evidence-based treatments.
Corey is married and has 2 children with whom he enjoys spending his time most. He loves being outdoors, learning, and riding his Harley. Corey is licensed to practice in Texas and Utah and has experience using Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI) and Solution-Focused Therapy (SFT). He is currently accepting new clients through Affinity Healtcare’s online platform, allowing clients the accessibility and convenience to meet virtually.
